• Undergraduate school, degree, location, dates
• A brief explanation in an attached cover letter explaining any unexplained gaps of time in the above history, if any

Professional Associations

• A listing of all organizations such as the American Medical Association to which you now belong or once belonged, the dates of your membership and current status of your membership

Medical Licenses & Board Certifications

• A complete listing of all state and country medical licenses you do now or have ever held, including the date of your licensure, license number(s),and the current status of each license (generally active, inactive and/or unrestrictive)
• A complete listing of all state and country board certifications you do now or have ever held, including the date of your certification, its current status and any certification number(s)
• Your USMLE board certification(s), including dates and scores
• DEA license status and number

Research & Publications
